By submitting this form, I understand that my meal plan will be cancelled and any meals eaten or Dining Dollars used will be deducted from my refund to Wolverine Access. If my meal plan is being canceled for winter term any remaining meals or funds from fall semester are forfeited. Any meals used in winter term before the cancellation of the winter term meal plan are subject to charge at the single meal rate and will be charged against the refund issued to Wolverine Access. *
